Good news for State Health Insurance Assistance Programs (SHIPs) around the country. The Centers for Medicare & Medicaid Services (CMS) is in the process of distributing nearly $46.5 million in basic grant funding to help people with Medicare, and those who care for them, receive information and counseling about their health benefits and choices.

“The funding is going to 54 SHIP organizations around the country,” says Alan Weinstock, an insurance broker at MedicareSupplementPlans.com. “SHIPs provide counseling to Medicare beneficiaries.” And with the new preventive services available at no cost, the CMS is hoping people will take advantage of this opportunity to get an annual wellness visit.

Let’s leave the politics of healthcare reform to one side and focus on a proposal to change the law to allow free market competition between insurers in different states. A policy consistently mentioned by the Republican party is to break the state monopolies in the insurance market. Since the 1800′s, the individual states have claimed the sole right to regulate the sale of insurance within their own borders. Each state has asserted the right to license insurance companies and to set the terms on which they can conduct business. This has led to a patchwork of different sets of regulations with each state creating unique laws. In turn, this forces an insurance company to set up separate subsidiaries to trade in each state. No licensed company can sell a policy to someone who has a residence in another state. There was a brief moment in 1944 when a decision of the Supreme Court allowed the possibility of federal supervision. But the lawmakers in Washington immediately changed the law to retain state control. Why is this a bad thing? The national insurance companies have divided up the states between them and choose not to compete against each other. This keeps the number of insurance companies in each state artificially low and, because there is no real competition, premium rates are higher than they should be for weak policy terms.

You are reading this article on the internet. When online, you can buy more or less any product or service across state or national boundaries. Although there are some restrictions, e.g. some states limit your right to import drugs from foreign countries, there is an almost free market where you can search for the cheapest price and buy whatever you need. There is no possible economic justification for retaining this historical privilege for insurance companies. All it does is preserve their ability to maximize their profits at your expense. For example, in Minnesota three insurance companies dominate 80% of the market for health plans. There is no doubt that, if more companies entered the market, the premium rates would fall. During his run for President, Senator John McCain was in favor of free markets for health plans. President Obama supports it and the proposal is in both versions of the healthcare reform bills currently stalled in Washington. But because the Republican party’s only policy is to oppose everything the Democrats propose, it seems even this simple change in the law may be lost. What will the result be? The anticompetitive behavior of the insurance industry will continue and you, the consumer, will suffer.

President Barrack Obama while explaining the concept of health insurance Exchange stated that it would be, “a market where Americans can one-stop shop for a health care plan, compare benefits and prices, and choose the plan that’s best for them, in the same way that Members of Congress and their families can. None of these plans should deny coverage on the basis of a preexisting condition, and all of these plans should include an affordable basic benefit package that includes prevention, and protection against catastrophic costs. I strongly believe that Americans should have the choice of a public health insurance option operating alongside private plans. This will give them a better range of choices, make the health care market more competitive, and keep insurance companies honest.”

Though that encapsulates everything good about these Exchanges, these state based online health insurance platforms are not without their drawbacks.

It is anticipated that the state health insurance Exchanges will forever change the way Americans look at their health policies. But to say that the Exchanges have everything right going for them would be far-fetched statement. The Exchanges are sure to bring about a drastic change in the world of health insurance, but the government might end up facing a few hurdles before it effectively implements these state based online insurance marketplaces.

The governance of the Exchange is a major concern. The Exchange will be funded by federal dollars. It has to meet the guidelines set by the federal as well as the state government. So should it governed by a body that represents both the parties? Or should a non-profit entity be handed over the charge of administration? Governing a regulated insurance marketplace, and more importantly, who is in charge of the job will perhaps be one the first few problems that the Exchange has to deal with.

It will provide relief to families who have no insurance or do not get adequate insurance at work and cannot afford to buy it in the costly individual or small group market. It is also for small businesses that cannot afford small group health insurance. It is one-stop shopping that will enable you and your family to find a plan that is right for you.’

This sums up the basic purpose behind establishing a health Exchange. However providing affordable healthcare is not the only reasons why millions of dollars are being invested in these state based online platforms. The Exchange will have several other functions as well, each of which is a step towards bringing more regulations to the health insurance sector.
